The art and science of dental implants has advanced tremendously in the past two decades. Patients who previously had to be condemned to wearing loose fitting dentures now can often have their missing teeth replaced with implants. Healthy teeth do not always have to be cut down to install dental bridges because a missing tooth can now be replaced with dental implants! There are many reasons why a patient should choose this option.
In the past, many patients were refused this procedure because of a lack of adequate bone. Today, bone grafting (Dental implant bone grafting is a surgical procedure that can usually be done in the dental office. It replaces the bone that has been lost with material from the patient's own body. It means that autogenous bone grafting material used to repair a defect came from the same patient that was receiving the bone graft. Basically, some bone is taken from one area of the body and transferred to another area of the same patient's body.) or with a natural, artificial, or synthetic substitute. In most cases the goal of the German implant dentist is to generate new healthy and natural bone in the area of the procedure. Some synthetic materials simply hold the space under the gums and the patient's body does not replace the material with natural bone.
Sinus Grafting
The maxillary sinuses are behind your cheeks and on top of the upper teeth. Sinuses are like empty rooms that have nothing at all in them. Some of the roots of the natural upper teeth extend up into the maxillary sinuses. When these upper teeth are removed, there is often just an egg shell thin wall of bone separating the maxillary sinus and the mouth. Dental implants need bone to hold them in place so when the sinus wall is very thin, it is impossible to place dental implants in this bone.
There is a solution and it's call a sinus graft or sinus lift graft. The dental implant surgeon enters the sinus from it's side above where the upper teeth used to be. The sinus membrane is then lifted upward and donor bone is inserted into the floor of the sinus. Remember, the floor of the sinus is the roof of the upper jaw. After several months of healing, the bone becomes part of the patient's jaw and dental implants can be inserted and stabilized in this new sinus bone.
The sinus graft makes it possible for many patients to have dental implants when years ago there was no other option other than wearing loose dentures. Dental Implants
A dental implant goes into the jaw bone like a natural tooth root goes into the jaw bone. Look at the dental implant diagram above and compare the dental implant to the natural tooth root. However, the dental implant is made out of titanium metal and the natural tooth root is made out of dentin with a coating on its outside called cementum.
The dental implant post precisely connects to the top of the dental implant. The dental implant post is usually screwed into the top of the dental implant. An artificial dental crown can be made to precisely fit onto the implant post. Most patients prefer natural looking porcelain fused to gold crowns or the state of the art full-ceramic or Zirkonia-Crowns, so that the artificial dental crown can cosmetically blend into the mouth.
The gums around the dental implant hug the top of the dental implant. The top of the dental implant is highly polished so that the gums can grow tightly to the polished surface and seal out food and debri.
The jaw bone around the dental implant holds it firmly in the mouth. The pressure from chewing on the implant crown is transmitted to the underlying bone and helps keep this bone healthy. Bone needs stimulation or it will melt away.
In this diagram there is an artificial crown placed on top of the dental implant post. This closely resembles the way natural intended teeth to function. Some patients prefer to attach a bar to several implants so that a denture can firmly clip to the bar. This is a less expensive use of dental implants and for many patients, helps correct their loose dentures problem. The dental implant show above is called a root form dental implant, however, a blade/plate form dental implant is available.
